Your diet over many years is often what has created the problem, so the changes you make may need to be kept in place for some time. This is where the lining of the intestine becomes more porous than usual. This may allow waste matter such as bacteria, fungus, parasites and their toxins, and undigested protein to enter your bloodstream. Having these toxins in the blood can cause allergies and multiple chemical sensitivity. There are a few different reasons why this can happen. It may be any of the following including, toxins such as heavy metals or chemicals from your environment present in the gut, impacted fecal matter that may be caused by constipation, or a diet high in sugar causing candida over growth. You may put them back into your diet one by one until you know what the problem foods are, those that trigger a bloated stomach, pain or any of the other common symptoms of leaky gut disorder. Adding positive helpful healing foods is also a major part of the leaky gut diet, and these are listed below. Leaky Gut Diet .. List Of Steps To Take. So to make it easy, follow the steps listed. Healing a leaky gut requires that you do a few things in tandem. Avoid foods containing gluten and take out as much processed food as possible. You may find that you can add back foods that contain gluten once you have the problem under control. Eat fibre daily - either as fresh fruit or by adding a non gluten fibre cereal to your daily diet. There are a number of benefits of apple cider vinegar and this may be added as a remedy and/or as a food, such as making it into a salad dressing. Drink more water, and particularly use clean filtered water. There are many benefits of drinking more water, and it is important to drink a good amount of water each day. Take remedies such as: Slippery Elm. L- glutamine Vitamin CMSM powder Digestive enzymes in a supplement form. Natural probiotics: Probiotics should taken on an empty stomach to restore gut flora. Leaky Gut Diet.. Food To Decrease or Eliminate. There are some specific foods that are known to create problems so these should be eliminated. Specifically eliminate: Gluten: contained in wheat, spelt, barley and rye. Oats also contains gluten but this is tightly bound gluten and may be tolerated. There is a variety of oats which is said to be gluten free, which may be worth trying. Ordinary oats: eliminate it to begin with then add it back carefully if you want. Note: There are a number of hidden sources of gluten, especially in sauces. On the label they may be called dextrin or modified starch. Anything that has thickening in it may contain gluten, so look for gluten free on labels. Refined sugar, sugary drinks, artificial sweeteners and alcohol. Sugary drinks are major contributors to the problem and the diet drinks are just as bad as they contain chemicals that irritate the gut. Coffee, tea and chocolate. Avoid processed food and cut back salt. Peanuts and tomatoes: these are ones that are likely to be problems so eliminate them to begin with and add back once you are sure you have your diet controlled. If you can do not add them back until you are definitely feeling well again as this might set back your progress. Beef, Dairy Produce and Soy. Avoid raw food containing phytic acid as it binds with nutrients to create deficiencies. Phytic acid is in wheat, soybeans, raw peanuts and other raw nuts. It is also in corn but corn contains folate and beta cryptoxanthin which help the health of the lungs, so you may decide to eat it or not. Soaking soybeans, nuts and other foods that are high in phytic acid for around 1. This will make the enzymes in the food more easily available and more easily digestible. As these are foods that contain zinc as well as being foods that are rich in magnesium they are useful to add to your diet. Once these raw foods have been made into products such as soy or almond milk the level of phytic acid should be low enough to ensure it is no longer a problem. Leaky Gut Diet.. Foods to Increase. It is a good idea to eat as many of the foods that are known to be low risk of allergies. The best low allergy foods to start with are: Rice milk or almond milk if you are not allergic to almonds. Herbal tea: take care with this and use herbal tea that you know is safe until you are sure you won't react to specific types. Lamb. Pears and apples: some people may be allergic to apples, but they are great if you are not, as apple pectin is excellent to aid the gut. Switch to eating organic food as there are advantages of organic food, and look at information about organic food to see more about the benefits of eating organic food. Beans and legumes except peanuts. Organic vegetables: lightly cook them to begin with to take the pressure off the gut digesting them. You may prefer to drink vegetable juice but eat vegetables too, for the fibre component in your diet. Then as you improve add in more raw food. Rice, millet, amaranth, quinoa and buckwheat. Pear juice or nectar. Once you have stabilized your health you may add in some of the foods known to aid the healing of the gut. Do this in line of course with any known allergies, do not eat any of these if you known you react to them, and add them in one at a time. Start your day by drinking lemon water, the juice of half a lemon in warm or tepid water not cold water. Vegetables: cabbage, lentils, fresh ginger, avocados and Jerusalem artichokes all have enzymes in them that aid the gut to heal. Fresh juice and aloe vera juice: which has a healing action on the gut. Fresh organic fruits: avoid fruits that are high in salicylates if you have previously reacted to salicylate, to take pressure off your immune system. Oily fish: such as tuna, sardines, mackerel, herring, anchovies. Olive oil or omega 6 oil. Organic apple cider vinegar. Onions and garlic: fresh garlic has a healing action, but use with care if you react to it at all as it has a strong action. Organic chicken. Seafood: make sure you avoid any seafood you know you may have allergic reactions to. Create A Safe Healing Environment If you have not already done it, have someone help you to make sure that your environment is safe by removing clutter from your home. It is important that you make sure that your living environment is safe too. As many of the problems in the gut are known to be caused by heavy metals and chemicals in your gut, it may not be enough to simply follow the leaky gut diet. If you have allergies to any products be doubly sure to avoid contact with them and to use unscented and low chemical products as much as possible. Once you have stabilized your health, ensure you keep healthy by continuing to use organic food and drinking lots of water. The leaky gut diet is a prominent part of healing from this condition, as it is commonly partly caused by long- term incorrect dietary choices. Eat slowly, take small bites and savor your food, this will aid your digestion. Use a diet diary to record what you have eaten, and you will find it easier to keep your body on the road to healing. Some pages that you may find helpful.. Are you looking for information about something in particular? Use this handy search box to see if its written about on this site.. What Should You Eat To Heal a Leaky Gut? It is also important to make sure you are getting enough Vitamin D. Eat foods that restore gut microflora. While these dietary changes may seem overwhelming, it is important to remember that making them will keep you healthy, put many diseases into remission, and prevent dozens of other diseases from developing. The Best Amazon Deals We Could Find Today, May 1. Jacques Mattheij made a small, but awesome, mistake. He went on e. Bay one evening and bid on a bunch of bulk LEGO brick auctions, then went to sleep. Upon waking, he discovered that he was the high bidder on many, and was now the proud owner of two tons of LEGO bricks. Much of the value of the bricks is in their sorting. If he could reduce the entropy of these bins of unsorted bricks, he could make a tidy profit. While many people do this work by hand, the problem is enormous—just the kind of challenge for a computer. Mattheij writes: There are 3. In the following months, Mattheij built a proof- of- concept sorting system using, of course, LEGO. Here’s what caught our eye today, May 16. Mental Floss has. He broke the problem down into a series of sub- problems (including . After tinkering with the prototype at length, he expanded the system to a surprisingly complex system of conveyer belts (powered by a home treadmill), various pieces of cabinetry, and . That allows the computer (when sufficiently trained on brick images) to recognize bricks and thus categorize them by color, shape, or other parameters. Das “Ristorante Da Calogero” ist eines der am längst bestehenden italienischen Restaurants in Quickborn, Schleswig Holstein. Seit Jahren offeriert Gastronom. Remember that as bricks pass by, they can be in any orientation, can be dirty, can even be stuck to other pieces. So having a flexible software system is key to recognizing—in a fraction of a second—what a given brick is, in order to sort it out. When a match is found, a jet of compressed air pops the piece off the conveyer belt and into a waiting bin. After much experimentation, Mattheij rewrote the software (several times in fact) to accomplish a variety of basic tasks. At its core, the system takes images from a webcam and feeds them to a neural network to do the classification. Of course, the neural net needs to be . Mattheij's breakthrough was allowing the machine to effectively train itself, with guidance: Running pieces through allows the system to take its own photos, make a guess, and build on that guess. As long as Mattheij corrects the incorrect guesses, he ends up with a decent (and self- reinforcing) corpus of training data. As the machine continues running, it can rack up more training, allowing it to recognize a broad variety of pieces on the fly. Here's another video, focusing on how the pieces move on conveyer belts (running at slow speed so puny humans can follow). You can also see the air jets in action. In an email interview, Mattheij told Mental Floss that the system currently sorts LEGO bricks into more than 5. It can also be run in a color- sorting mode to bin the parts across 1. At some point down the line, he plans to make the software portion open source. You're on your own as far as building conveyer belts, bins, and so forth. Check out Mattheij's writeup in two parts for more information. It starts with an overview of the story, followed up with a deep dive on the software. He's also tweeting about the project (among other things). And if you look around a bit, you'll find bulk LEGO brick auctions online—it's definitely a thing! Anti- androgen therapy . Male Infertility Overview Assessment, Diagnosis, and Treatment. Shaban, M.D. Clinical Assistant Professor Department of Surgery, Division of Urology. What is anti- androgen therapy? Anti- androgen therapy refers to medication taken by women to counteract the effect of male sex hormones such as testosterone on the skin. Anti- androgens are not suitable for skin problems in men. What are anti- androgens used for? Anti- androgen medications are used to treat signs of hyperandrogenism, including the following skin and hair disorders: How does anti- androgen therapy work? Anti- androgen therapy may: Block androgen receptor. Reduce adrenal androgen production. Reduce ovarian androgen production. Reduce pituitary production of prolactin. Inhibit 5- alpha reductase (this enzyme acts in the skin to increase dihyroxytestosterone)Reduce insulin resistance. Which drugs have anti- androgenic action? Androgen receptor blockers act on the sebaceous gland and base of the hair follicle. They include: Oral contraceptive (birth control pill) containing ethinylestrodiol (oestrogen) and an antiandrogenic progesterone. These include cyproterone acetate (co- cyprindiol or Diane. These contain ethinylestrodiol and desorgestrel, gestodene or norgestimate. Spironolactone 2. Aldactone. Cyproterone acetate 5. Androcur. This powerful anti- androgen is usually taken on days 1 to 1. Flutamide 2. 50- 5. This is normally used as a hormonal antineoplastic agent in males with prostate cancer. It can cause hepatitis and should not be used for skin disorders. Spironolactone and cyproterone may be effectively combined with cyproterone acetate/ethinyloestradiol or other oral contraceptive agent, partly because they cause menstrual irregularities and partly to prevent pregnancy. The combined treatment is not necessary in post- menopausal women. Low- dose oral corticosteroid (eg prednisone 2. In congenital adrenal hyperplasia, DHEAS levels should reduce to normal. Limecycline, roxithromycin and ketoconazole (no longer available in New Zealand) are antimicrobial drugs that have also been noted to reduce androgen synthesis. Drugs acting on ovarian androgen production include: Gonadotrophin receptor hormone (Gn. RH) agonist (buserelin, leuprolide), which stop ovulation and suppress androgen production. Because they also stop oestrogen production, they may lead to menopausal symptoms, headache and osteoporosis. Combined oral contraceptives. Progestins. Excessive prolactin is reduced by bromocriptine, cabergoline and quinagolide. Spironolactone inhibits 5- alpha reductase weakly. Unfortunately, finasteride does not reduce sebum production and is not effective in the treatment of acne. However, we now know that isotretinoin reduces sebum partly by reducing dihyrotestosterone production in the sebaceous gland. Insulin resistance can be reduced using metformin, mainly prescribed for type 2 diabetes mellitus and obesity / metabolic syndrome. It may also reduce signs of hyperandrogenism. Metformin 2. 50 mg to 2 g daily is safe but can cause diarrhoea and should be taken after food in gradually increasing doses. Rosiglitazone and pioglitazone can cause heart and liver toxicity. What are the clinical effects of anti- androgen therapy? In acne, the effects of anti- androgens include: They can be combined with other topical and oral treatments for acne. In hirsutism, the results are: Slower growth of hair. Lighter coloured hair. Finer textured hair. Physical methods of hair removal such as waxing, shaving, electrolysis or laser epilation, can be used at the same time as anti- androgens are taken. They often work better than prior to the medication. In female pattern hair loss, the results are: Reduced hair shedding. Reduced hair thinning. Sometimes, restoration of thicker hair. These effects are not always clinically significant. Oral contraceptives. Progesterone- only oral contraceptives are not effective in the management of androgen- mediated skin conditions. Combined oral contraceptives contain two hormones, ethinyloestrodiol 2. They prevent pregnancy by suppressing ovulation and changing cervical mucus. There are various kinds of progesterone, which may be androgenic in nature and thus unsuitable for those androgen- mediated skin conditions (particularly levonorgesterol and norgestrel). Anti- androgenic or minimally androgenic progesterones (see above) are indicated in these women. Their effect in hyperandrogenism is to reduce production of androgens by the ovaries, by the adrenals and at the receptor level in the skin. They also decrease circulating testosterone by increasing sex hormone binding globulin (SHBG). Combined oral contraceptives are available as 2. Day 1 (conventionally, day 1 is the first day of menstruation) and take one a day for 2. Then have a 7- day break (2. During this time, you can expect a withdrawal bleed (a period). Combined oral contraceptives can increase the risk of thromboembolism (blood clots blocking blood vessels), especially in those with an inherited tendency (. Please refer to the New Zealand Ministry of Health (Medsafe) advice on the use of combined oral contraceptives. The combined oral contraceptive may be unsuitable if the patient: Has had a previous blood clot (thrombosis, embolism or . They may occasionally increase the risk of certain uncommon forms of breast cancer. They must not be taken in pregnancy. On the other hand, the combined oral contraceptive reduces the risk of ovarian and endometrial cancer, benign breast disease, ectopic pregnancy, painful periods, iron deficiency anaemia and pelvic inflammatory disease. Cyproterone acetate/ethinyloestradiol (co- pyrindiol) should be discontinued in the following circumstances: Severe migraine (headache, visual disturbance, numb feelings)Any form of thrombosis (such as heart attack, stroke) or increased blood pressure. In case of immobility (including due to surgery) or trauma (eg bad sprains)If an increase in number or severity of epileptic seizures occurs, or liver disease arises during treatment. Pregnancy. As with other oral contraceptives, minor side effects may arise, especially in the first few weeks. These include: Breast tenderness (2. Rarely, periods may stop altogether (amenorrhoea). If you have spotting (bleeding in between periods), see your doctor. He or she may advise you to take the active medication continuously for 3 months or more. This is quite safe and often prevents unwanted bleeding. Nausea, loss of appetite and bloating. Headaches. Increased appetite and weight increase (uncommon)Mood changes including depression and reduced libido Melasma (facial pigmentation)Hair loss (it is more common for hair fall to occur when the medication has been discontinued however)Acne. Other medications can interfere with the contraceptive effectiveness. Combined oral contraceptives with anti- androgenic components have advantages: They regulate the menstrual cycle in the majority of women. Lighter, less painful periods occur in the majority of women. Iron deficiency anaemia is less common because of less bleeding. Increased . Combined oral contraceptives can usually be taken safely for many years. Unfortunately, the skin condition tends to deteriorate again within a few months after the medication has been stopped. New forms of oral contraceptive are introduced from time to time, to increase efficacy and reduce side effects. Please refer to the New Zealand Ministry of Health (Medsafe) advice on the use of combined oral contraceptives. Spironolactone. Spironolactone is a potassium- sparing medication used as a diuretic medication for heart failure, liver disease and high blood pressure. However, it has also been found useful for hirsutism, acne and seborrhoea because it has anti- androgenic properties. Spironolactone mainly works by blocking androgen receptors. The dose of spironolactone is usually slowly increased from 2. It is sometimes prescribed cyclically to reduce menstrual irregularities, eg, for 3 weeks out of every 4 weeks or days 5–2. It may take six or more months to see improvement in the skin condition. Side effects of spironolactone include: Nausea, diarrhoea, gastrointestinal bleeding. Drowsiness (possibly due to high blood potassium levels)Headache and dizziness. Menstrual cycle irregularities including increased frequency of menstruation, bleeding in the middle of the month or stopping periods altogether (amenorrhoea)Breast pain. Potassium, other electrolytes and creatinine levels in the blood are often monitored. This is especially necessary in older women, if high doses are prescribed, in patients taking other medicines (due to drug interactions) and in those with heart or kidney problems. Spironolactone should not be taken in pregnancy or during lactation. Serum potassium should be . Cyproterone acetate. Higher doses of cyproterone acetate are indicated for more severe cases of androgenetic skin conditions. It is effective for 7. Several different regimes are prescribed with doses ranging from 2. Prior to the menopause, the medication is usually combined with cyproterone acetate/ethinyloestradiol or other oral contraceptive agent: To regulate menstrual cycle irregularities caused by the high dose cyproterone. To prevent pregnancy; there are concerns that cyproterone could harm a male fetus by . It may be advisable to have a 7- day break every month. They may also take spironolactone. Occasional significant side effects include: Liver disturbance. Tiredness and depression. Weight increase. Related information. Male Infertility Overview. Male Infertility Overview. Assessment, Diagnosis, and Treatment. Stephen F. Shaban, M. D. Clinical Assistant Professor. Department of Surgery, Division of Urology. University of North Carolina School of Medicine. Chapel Hill, NC. Most authorities define these patients as primarily infertile if they have been unable to achieve a pregnancy after one year of unprotected intercourse. Conception normally is achieved within twelve months in 8. Data available over the past twenty years reveal that in approximately 3. Therefore, the male factor is at least partly responsible in about 5. It is extremely important in the evaluation of infertility to consider the couple as a unit in evaluation and treatment and to proceed in a parallel investigative manner until a problem is uncovered. It has been shown that the longer a couple remains subfertile, the worse their chance for an effective cure. Many couples experience significant apprehension and anxiety after only a few months of failure to conceive. Unduly prolonged unprotected intercourse should not be advocated before a workup of the man is instituted. Initial screening of the man should be considered whenever the patient presents with the chief complaint of infertility. This initial evaluation should be rapid, non- invasive and cost effective. Of interest is the fact that pregnancy rates of up to 5. MALE REPRODUCTIVE PHYSIOLOGYThe Hypothalamic- Pituitary- Gonadal Axis. The hypothalamus is the integrative center of the reproductive axis and receives messages from both the central nervous system and the testes to regulate the production and secretion of gonadotropin releasing hormone (Gn. RH). Neurotransmitters and neuropeptides have both inhibitory and stipulatory influence on the hypothalamus. The hypothalamus releases Gn. RH in a pulsatile nature which appears to be essential for stimulating the production and release of both luteinizing hormone (LH) and follicle stimulating hormone (FSH). Interestingly and paradoxically, after the initial stimulation of these gonadotropins, the exposure to constant Gn. RH results in inhibition of their release. LH and FSH are produced in the anterior pituitary and are secreted episodically in response to the pulsatile release of Gn. RH. LH and FSH both bind to specific receptors on the Leydig cells and Sertoli cells within the testis. Testosterone, the major secretory product of the testes, is a primary inhibitor of LH secretion in males. Testosterone may be metabolized in peripheral tissue to the potent androgen dihydrotestosterone or the potent estrogen estradiol. These androgens and estrogens act independently to modulate LH secretion. The mechanism of feedback control of FSH is regulated by a Sertoli cell product called inhibin. Decreases in spermatogenesis are accompanied by decreased production of inhibin and this reduction in negative feedback is associated with reciprocal elevation of FSH levels. Isolated increased levels of FSH constitute an important, sensitive marker of the state of the germinal epithelium. In males with hyperprolactinemia, the prolactin tends to inhibit the production of Gn. RH. Besides inhibiting LH secretion and testosterone production, elevated prolactin levels may have a direct effect on the central nervous system. In individuals with elevated prolactin levels who are given testosterone, libido and sexual function do not return to normal as long as the prolactin levels are elevated. The Testes. Leydig; Cells. Testosterone is secreted episodically from the Leydig cells in response to LH pulses and has a diurnal pattern, with the peak level in the early morning and the trough level in the late afternoon or early evening. In the intact testis, LH receptors decrease or down- regulate after exogenous LH administration. Large doses of Gn. RH or its analogs can reduce the numbers of LH receptors and therefore inhibit LH secretion. This has been applied clinically to cause medical castration in men with prostate cancer. Estrogen inhibits some enzymes in the testosterone synthetic pathway and therefore directly effects testosterone production. There also appears to be an intratesticular ultra short loop feedback such that exogenous testosterone will override the effect of LH and inhibit testosterone production. In normal males, only 2% of testosterone is free or unbound. Te. BG, also called sex hormone- binding globulin. These steroid- binding proteins modulate androgen action. Te. BG has a higher affinity for testosterone than for estradiol, and changes in Te. BG alter or amplify the hormonal milieu. Te. BG levels are increased by estrogens, thyroid administration and cirrhosis of the liver and may be decreased by androgens, growth hormone and obesity. The biological actions of androgens are exerted on target organs that contain specific androgen receptor proteins. Testosterone leaves the circulation and enters the target cells where it is converted to the more potent androgen dihydrotestosterone by an enzyme 5- alpha- reductase. The major functions of androgens in target tissues include 1) regulation of gonadotropin secretion by the hypothalamic- pituitary axis; 2) initiation and maintenance of spermatogenesis; 3) differentiation of the internal and external male genital system during fetal development; and 4) promotion of sexual maturation at puberty. Seminiferous Tubules. The seminiferous tubules contain all the germ cells at various stages of maturation and their supporting Sertoli cells. These account for 8. Sertoli cells are a fixed- population of non- dividing support cells. They rest on the basement membrane of the seminiferous tubules. They are linked by tight junctions. These tight junctions coupled with the close approximation of the myoid cells of the peritubular contractile cell layers serve to form the blood- testis barrier. This barrier provides a unique microenvironment that facilitates spermatogenesis and maintains these germ cells in an immunologically privileged location. This isolation is important because spermatozoa are produced during puberty, long after the period of self- recognition by the immune system. If these developing spermatozoa were not immunologically protected, they would be recognized as foreign and attacked by the body's immune system. Sertoli cells appear to be involved with the nourishment of developing germ cells as well as the phagocytosis of damaged cells. Spermatogonia and young spermatocytes are lower down in the basal compartment of the seminiferous tubule, whereas mature spermatocytes and spermatids are sequestered higher up in the adluminal compartment. Spermatogonia lie directly on the basement membrane, and next in order, progressing up to the lumen, are found the primary spermatocytes, secondary spermatocytes and spermatids. There are felt to be 1. The spermatocytes eventually divide and give rise to mature cell lines that eventually give rise to spermatids. The spermatids then undergo a transformation into a spermatozoa. This transformation includes nuclear condensation, acrosome formation, loss of most of the cytoplasm, development of a tail and arrangement of the mitochondria into the middle piece of the sperm which basically becomes the engine room to power the tail. Groups of germ cells tend to develop and pass through spermatogenesis together. This sequence of developing germ cells is called a generation. These generations of germ cells are basically in the same stage of development. There are six stages of seminiferous epithelium development. The progression from stage one through stage six constitutes one cycle. In humans the duration of each cycle is approximately 1. Therefore, the duration of the entire spermatogenic cycle in humans is 4. LH effects spermatogenesis indirectly in that it stimulates androgenous testosterone production. FSH targets Sertoli cells. Therefore, testosterone and PSH are the hormones that are directed at the seminiferous tubule epithelium. Androgen- binding protein which is a Sertoli cell product carries testosterone intracellularly and may serve as a testosterone reservoir within the seminiferous tubules in addition to transporting testosterone from the testis into the epididymal tubule. The physical proximity of the Leydig cells to the seminiferous tubules and the elaboration by the Sertoli cells of androgen- binding protein, cause a high level of testosterone to be maintained in the microenvironment of the developing spermatozoa. The hormonal requirements for initiation of spermatogenesis appear to be independent of the maintenance of spermatogenesis. For spermatogenesis to be maintained like for instance after a pituitary obliteration, only testosterone is required. However, if spermatogenesis is to be re- initiated after the germinal epithelium has been allowed to regress completely, then both FSH and testosterone are required. Testicular spermatozoa are non- motile and were felt to be incapable of fertilizing ova. Spermatozoa gain progressive motility and fertilizing ability after passing through the epididymis. The coiled seminiferous tubules terminate within the rete testis, which in turn coalesces to form the ductuli efferentes. These ductuli efferentes conduct testicular fluid and spermatozoa into the head of the epididymis. The epididymis consists of a fragile single convoluted tubule that is 5- 6 meters in length. The epididymis is divided into the head, body, and tail. Although epididymal transport time varies with age and sexual activity, the estimated transit time of spermatozoa through the epididymis in healthy males is approximately four days. It is during the period of maturation in the head and body of the epididymis that the sperm develop the increased capacity for progressive motility and also acquire the ability to penetrate oocytes during fertilization. The epididymis also serves as a reservoir or storage area for sperm. It is estimated that the extragonadal sperm reservoir is 4. Bariatric Vitamins, Gastric Bypass Vitamins & Bariatric Proteins. Gastric bypass diet: What to eat after the surgery. Definition. A gastric bypass diet is for people who are recovering from gastric bypass surgery to help them heal and change their eating habits. Gastric bypass surgery is one of several weight- loss surgeries currently performed. The operation itself has undergone several modifications over the years. The procedure in use today is called the Roux- en- Y gastric bypass. It shouldn't be confused with other weight- loss surgeries, such as the biliopancreatic diversion with duodenal switch, which is a more aggressive surgery. Your doctor or a registered dietitian will talk with you about the diet you'll need to follow after surgery, explaining what types of food and how much you can eat at each meal. Closely following your gastric bypass diet can help you lose weight safely. Purpose. The gastric bypass diet has several purposes: To allow your stomach to heal without being stretched by the food you eat. To get you used to eating the smaller amounts of food that your smaller stomach can comfortably and safely digest. To help you lose weight and avoid gaining weight. To avoid side effects and complications from the surgery. Diet details. Diet recommendations after gastric bypass surgery vary depending on where the surgery is performed and your individual situation. A gastric bypass diet typically follows a staged approach to help you ease back into eating solid foods as you recover. How quickly you move from one step to the next depends on how fast your body heals and adjusts to the change in eating patterns. You can usually start eating regular foods about three months after surgery. After gastric bypass surgery, you must be careful to drink enough fluids to avoid dehydration, and to pay extra attention to signs that you feel hungry or full. Liquids. For the first day or so after surgery, you'll only be allowed to drink clear liquids. Sip slowly and drink only 2 to 3 ounces (5. Once you're handling clear liquids, you can start having other liquids, such as skim or low- fat milk. Liquids you can have during stage 1: Broth. Unsweetened juice. Decaffeinated tea or coffee. Milk (skim or 1 percent)Strained cream soup. Sugar- free gelatin or popsicles. Pureed foods. Once you're able to tolerate liquids for a few days, you can begin to eat strained and pureed (mashed up) foods. During this phase, you can only eat foods that have the consistency of a smooth paste or a thick liquid, without any solid pieces of food in the mixture. To puree your foods, choose foods that will blend well, such as: Lean ground meats. Beans. Fish. Eggs Soft fruits and cooked vegetables. Cottage cheese. Blend solid foods with a liquid, such as: Water. Skim milk. Juice with no sugar added. Broth. It's important that you don't eat and drink at the same time. Wait about 3. 0 minutes after a meal to drink anything. Also keep in mind that your digestive system might still be sensitive to spicy foods or dairy products. If you'd like to eat these foods during this phase, add them into your diet slowly and in small amounts. Soft foods. After a few weeks of pureed foods, and with your doctor's OK, you can add soft foods — in the form of small, tender, easily chewed pieces — to your diet. During this stage, your diet can include: Ground or finely diced meats. Canned or soft fresh fruit (without seeds or skin)Cooked vegetables (without skin)Solid foods. After about eight weeks on the gastric bypass diet, you can gradually return to eating firmer foods. But foods must still be chopped or diced. Start slowly with regular foods to see what foods you can tolerate. You may find that you still have difficulty eating spicier foods or foods with crunchy textures. Even at this stage, there are foods you should avoid because they may cause gastrointestinal symptoms, such as nausea, pain or vomiting. Foods to avoid: Nuts and seeds. Popcorn. Dried fruits. Carbonated beverages. Granola. Stringy or fibrous vegetables, such as celery, broccoli, corn or cabbage. Tough meats or meats with gristle. Fried foods. Breads. Over time, you may be able to try some of these foods again, with the guidance of your doctor. A new healthy diet. Three to four months after weight- loss surgery, you may be able to start eating a normal healthy diet, depending on your situation and any foods you may not be able to tolerate. It's possible that foods that initially irritated your stomach after surgery may become more tolerable as your stomach continues to heal. Throughout the diet. To ensure that you get enough vitamins and minerals and keep your weight- loss goals on track, at each stage of the gastric bypass diet, you should: Eat and drink slowly. Eating or drinking too quickly may cause dumping syndrome — when foods and liquids enter your small intestine rapidly and in larger amounts than normal, causing nausea, vomiting, dizziness, sweating and eventually diarrhea. To prevent dumping syndrome, choose foods and liquids low in fat and sugar, eat and drink slowly, and wait 3. Take at least 3. 0 minutes to eat your meals and 3. Keep meals small. During the diet progression, you should eat several small meals a day and sip liquids slowly throughout the day (not with meals). You might first start with six small meals a day, then move to four meals and finally, when following a regular diet, decrease to three meals a day. Each meal should include about a half- cup to a cup of food. Make sure you eat only the recommended amounts and stop eating before you feel full. Drink liquids between meals. Expect to drink at least 6 to 8 cups (4. Drinking liquids with your meals can cause pain, nausea and vomiting as well as dumping syndrome. Also, drinking too much liquid at or around mealtime can leave you feeling overly full and prevent you from eating enough nutrient- rich food. Chew food thoroughly. The new opening that leads from your stomach into your intestine is very small, and larger pieces of food can block the opening. Blockages prevent food from leaving your stomach and can cause vomiting, nausea and abdominal pain. Take small bites of food and chew them to a pureed consistency before swallowing. If you can't chew the food thoroughly, don't swallow it. Focus on high- protein foods. Immediately after your surgery, eating high- protein foods can help you heal. High- protein, low- fat choices remain a good long- term diet option after your surgery, as well. Low- fat cheese, cottage cheese and yogurts also are good protein sources. Avoid foods that are high in fat and sugar. After your surgery, it may be difficult for your digestive system to tolerate foods that are high in fat or added sugars. Avoid foods that are high in fat (such as fried foods, ice cream and candy bars). Look for sugar- free options of soft drinks and dairy products. Try new foods one at a time. After surgery, certain foods may cause nausea, pain and vomiting or may block the opening of the stomach. The ability to tolerate foods varies from person to person. If a food causes discomfort, don't eat it. As time passes, you may be able to eat this food. Foods and liquids that commonly cause discomfort include meat, bread, raw vegetables, fried foods and carbonated beverages. Take recommended vitamin and mineral supplements. Because a portion of your small intestine is bypassed after surgery, your body won't be able to absorb enough nutrients from your food. You'll likely need to take a multivitamin supplement every day for the rest of your life. Talk to your doctor about what type of multivitamin might be right for you and whether you might need to take additional supplements, such as calcium. Feb. 2. 0, 2. 01. DIY Elimination Diet for Dog and Cat « Dr. Matt Allworth – Veterinarian. Dogs and cats with long- standing skin or gastrointestinal troubles may be suffering food allergy. Commercial laboratories offer vets a blood testing service with an accuracy of up to 6. Many vet dermatologists view them as . I don’t recommend wasting your money on blood tests; spend it on an venison instead. Have as few ingredients as possible: preferably one source of protein, and one source of carbohydrate. The more ingredients, especially if commercial foods are included, the greater the likelihood an allergen may be inadvertently included in the elimination diet, ending in a false negative result. Dogs should be fed a protein and carbohydrate that has rarely, if ever, been offered previously. Unusual protein sources are ideal, such as kangaroo, rabbit, duck, turkey, crocodile, or other game meats. Fish is a good choice as it’s also loaded with anti- allergy Omega- 3’s. The offal of the protein source can be fed, but not that of other species. Protein can be served cooked or raw. Unusual carbohydrates are potatoes, rice or pulses. An approximate recipe is 2 parts carbohydrate to 1 part protein. Protein content shouldn’t fall below 2. While it’s tempting to incorporate other veggies, they’re not necessary in a short term trial. Cats can receive a 1. Kangaroo, rabbit, and turkey are good choices. Offal of the chosen species is ok. If your cat is unwilling to eat the one species for the 6 to 8 week trial period, you may need to select 2 proteins, and alternate. In both cat and dog, religiously exclude ingredients commonly used in commercial pet food such as beef, chicken, lamb (for dogs), soy, dairy, fish (for cats), wheat, oats, and corn. Dairy exclusion should encompass lactose- free milk products, manufactured for pets. In young, rapidly growing animals, it’s best to consult with your regular vet regarding balancing a formulation, or using a specialised commercial diet. Ingredient lists can’t always be relied upon. Supermarket brands can contain a Noah’s Ark of unlisted protein and other ingredients. If life’s already busy enough, preparing meals for the family, and 2 months of homecooking for the pet isn’t feasible, there are a handful of commercial foods recommended by vets for an elimination trial. These cost 2. 0- 3. Hills Prescription Diet Z/d or D/d – rice/egg. Eukanuba Response Formula FP – fish/potato. Royal Canin Therapeutic, feline: pea/duck, pea/venison, pea/rabbit, pea/lamb, or Hypoallergenic HPRoyal Canin Therapeutic, canine: potato/duck, potato/venison, potato/rabbit, potato/whitefish, or Hypoallergenic HPIf however, your pets skin or tummy trouble fails to improve on these commercial diets, it is still possible a home- cooked, novel protein diet may be effective. Most food allergies are perennial and the flow of seasons offers no relief. However, when a pet suffers multiple allergies, and some seasonality is observed, conduct the elimination trial a time of year when the skin is usually bad, so a therapeutic benefit is easier to detect. This is important, to firm- up our confidence the diet actually worked, and identify the exact allergen(s). Understandably, once a food allergen is excluded and long- standing symptoms begin to clear, some pet owners are reluctant to enter a challenge phase. The trouble wih this approach is: Failure to identify the allergen, so all future dietary decisions are made in the dark. Fingering individual allergen(s) offers greater flexibility when selecting commercial foods and formulating homecooked recipes. Home cooked diets are often unbalanced. This is especially important in young, rapidly growing dogs, and cats living on 1. Having at least a small amount for commercial food in the daily ration is like giving a multivitamin. For more, see The Arguement for Dietary Diversity. Scientifically test for the allergen, using staged re- introduction of ingredients, one- by- one. It can take between 2 days and 2 weeks for symptoms for reappear after introducing a food allergen, so don’t add a new ingredient more frequently than one every 2 weeks. Test all the major allergens starting with beef, chicken and lamb, fish for cats, and then check dairy, eggs, and pasta for wheat. Cases of multiple allergy are rare, but if you discover one, continue to test the others. If no symptoms return, you have to question if the response to elimination diet was real, or if the skin problem spontaneously resolved, coincidentally. Once the allergen is fingered, you can start scrutinising labels and testing their claims wih your beef- detecting dog. More crudely, experiment by offering commerical diets. As these represent a pot- purri of potential allergens, listed on packaging and unlisted contaminants, this process is very hit- and- miss. When symptoms return, guess which is the offending ingredient. If you’re lucky, after 3 months of scouring butchers for exotic meats, boiling rice and potatoes, and getting dirty looks from a fussy cat, you may be liberated from the treadmill of derma- pharmaceuticals.
